INSTRUCTIONS
This questionnaire is designed to obtain information about the person you report to. Your answers will be treated in strict confidence.
VERY IMPORTANT:
In this questionnaire, you will find that statements refer to 'the person I report to'. This refers to your direct supervisor or manager in the organisation. The questionnaire consists of three sections.
The first section deals with information about you.
The second section deals with those things which the person you report to, often does in the workplace.
The third section consists of one question on how efficient you think the person you report to, is and a few questions on how you feel about change and trust in the organisation.
Please complete the survey in one session (cannot be book marked or saved and returned to later)
